TheCSIGuys - Required Documents

1.	Deposits are non-refundable and will be applied to the final cost of the party. Deposits must be accompanied by a credit card.
2.	There is a Fuel surcharge applied for parties over 10 miles outside the 85308 zip code. Total Fuel Surcharge: $_____.00
3.	The Deposit is a minimum of $75.00 and a credit card is required to hold the following date and time: ____/____/ 2008 @ ___:___ AM / PM
4.	In case of cancellation, you, the client, must inform us, The CSI Guys, 72 hours prior to the party. If less than 72 hours, the full party price of $_________ will be charged to the credit card securing your date and time.
5.	Rescheduling of a party must be made with a minimum of one week notice. If shorter than a week notice is given, a $50.00 fee will be assessed.
6.	Rescheduling is based on availability.
7.	Final payment must be made no less than 72 hours prior to the party.
8.	Any changes to the proposal, including selections, must be made one week prior to the party.
9.	You will be contacted one week prior to the party for final approval and to confirm date and time of party.
10.	The CSI Guys will be arriving at your home at least 60 minutes prior to the party starting for set-up.
11.	A space of 10 ft x 10 ft is required for crime scene set-up, an additional 8ft area is also required for lab station set-up.
12.	All participants must have the statement of understanding / photo release signed by a parent or guardian, otherwise they will not be able to participate.
13.	The crime scene will take approx 2 - 2 1/2 hours. At the completion of the crime scene, cake will be served and presents given. Clean-up will then take place. Total party time is 2 1/2 to 3 hours.
14.	The CSI Guys will provide, gloves, booties and all lab material needed. If any person participating has and allergies, especially to latex, the CSI Guys need to know during the final approval and confirmation call.
15.	One guardian of the birthday child must be present at all times.
16.	Everyone must have fun!
